IMPORTANT NOTE: Every effort is made to place only healthy dogs; however, TLR cannot guarantee the health of any dog

adopted from us .

• If, within the first two weeks after adoption, you notice any health-related issues with the dog, please inform TLR

about it immediately and before taking the dog to your vet

• You will need explicit agreement with TLR that the vet bills for this issue will be paid by TLR.

• After two weeks, TLR is not responsible for any medical costs for the dog

Regarding the adoption of the above-described dog from TRUE LOVE Dog Rescue, (hereafter called TLR), I, the undersigned, agree to the following conditions:

- To assume all legal risks and obligations of ownership, and I agree to hold TLR and its volunteers harmless for any loss of, damage to, or injury to persons, animals, or property arising from or relating to the.above dog.
- To keep this dog in my possession and not give away, sell, or abandon it. In the extraordinary circumstance that I am not able to keep the dog, I will surrender the dog to TLR.
- To provide a safe, clean environment with a securely fenced yard .
- To care for this dog humanely at all times and to provide necessary medical care.
- To update and maintain current address information in the microchip registry and to have a current license
- To allow a TLR representative to visit my home and the dog 24 hours after adoption and for up to two weeks after adoption of this dog.
- To strongly consider a basic obedience class in order to better know and bond with this dog
- To pay the requested adoption fee (payable to TLR). I understand that this adoption fee is a contribution towards TLR's efforts in caring for rescued dogs and is not refundable .
- To expect all available medical records for the dog from TLR upon TLR's receipt of the completed adoption contract and the paid fee.
- To give my permission for a TLR volunteer to remove this dog from my custody if this dog is being neglected, abused, or has not been altered as agreed and entry for this purpose shall not constitute a trespass upon the premises occupied .
